I Just Want To Be Sure Whether I Have RLS Or Peripheral Neuropathy Or Both ?
Question: Hello Doctor, since past 1.5 years i am facing constant tingling sensations in both lower side of feets accompanied by occasional twitches and lightness sensation in calf muscles however no pain, no numbness can be felt either. During initial days, it used to get worse in night so much that i had to use crepe bandage on both legs tightly in order to get sleep or just move my limbs all together to get relief, it was just restless buzzing tingly sensations. At that time , idiopathic RLS was the DX since i got no sugar,no vit. deficiency nor any thyroid issues, iron too normal. I was put on gabapentin which fixed all issues of night but mild daytime sensations persisted till today, I have got done my NCV test which says "Distal mild right motor axonal peroneal neuropathy" while i have symptoms in both feets,i can feel restlessness in my lower part of legs while sitting on chair as well, i suffer from stress/anxiety as well. I just want to be sure whether i have RLS Or Peripheral Neuropathy Or Both ??? Thanks

How can i figure out what's the cause of my peripheral neuropathy? Do you think if i take Pregabalin Sustained released variant, will work for 24 hours ? i have tried talking about SR variant with my doctor, but he says you will get daytime sleepiness if you use that, which offcouse i don't want.

You could try pregabalin SR variant. Not everyone gets daytime sleepiness. If you do get sleepy in daytime, you could always switch back to the current combination.
You have already done the tests for common conditions such as diabetes and vitamin B12 testing. Further testing is not needed at present. If your symptoms worsen (which is unlikely), then additional testing may be needed.

Thanks doctor, just for info i wanted to know whether pregabalin and gabapentin can be switched over without tapering? Is there any side-effect in that? does long term use affects any organ or brain in particular ?
Also, One of the neurologist recommended me some nutritional supplement called Palmiges Capsules for neuropathy ??? is it effective or cures neuropathy by any chance?? i haven't purchased it yet because its just too costly for 10 capsules, on top of that pregabalin itself cost a lot than gabapentin.

Is my condition too referred to as neuropathic pain ??? because technically i don't have any physical pain anywhere, its just constant tingling sensation, sometimes pins and needles sensation on and off? I'll purchase palmiges capsule, how will i know whether its working or not ??

Thanks a lot doctor for clarifying my doubts. Just one more query, i was reading that wonderful slide you made and shared, in that one of the slide you have mentioned that the medication such as gaba or pregaba prolonged use leads to de-sensitization of receptors so should I be concerned about it in any way ?? Because i am looking at long time use of these medication for my symptoms.
Also, please suggest any multi-vitamin or specific vitamin that can be helpful in my neuropathy?? i sometimes feel that i may have acquired neuropathy through toxic elements from air or water i drink so i guess i need an anti-oxidant as well.

Sorry, i forgot to ask in previous query, that whether this affects limb's finger because i feel tightness when i try to bend my right feet all fingers, and when i manually try to bend them ,it hurts bad. I feel as if the joint of each finger got swollen, i suffer from hyperuricemia at times but not gout, i keep it under control via medicine every other month. i thought i should let you know and clarify, whether it is related with this motor peroneal neuropathy since ncv too have highlighted about right leg distal motor axonal peroneal neuropathy only.
