Brief Answer:
Full functional recovery can take 12-16 weeks.

Detailed Answer:
Hello,
Thanks for posting your query.
Grade 3 AC separation usually heals with rest in 3-5 weeks. It does not need a surgery and usually a full recovery of shoulder function is seen in 12-16 weeks. In case proper healing does not take place conservatively then surgical reconstruction is considered.
Mild residual pain and instability of overhead movements can happen in some cases.
Strength related tasks may need to be avoided for 4-6 months. Regular physiotherapy is needed for proper recovery.
I hope this answers your query.
In case you have additional questions or doubts, you can forward them to me, and I shall be glad to help you out.
Wishing you good health.
Regards.
