I Need To Have Forms Completed So I Can Take

Question: I need to have forms completed so I can take my service dog on a flight. My dog is a emotional support/Psychiatric service animal. The forms require the name of the health professional, name of practice, type of medical license, date of medical license and state of jurisdiction, signature and date. Is this something you can provide. I can email or fax the forms.

This certification should be done by a Veterinarian who physically examines the dog.

Doctor, please read the form again specifically looking at the section "This section to be completed by a MENTAL HEALTH PROFESSIONAL. There is noting on the form to be completed by a veterinarian.

The form is to be completed by a mental health professional (not veterinarian) certifying the patient needs the above mentioned animal to travel as an emotional support of psychiatric service animal for air travel and/or for activity at their destination.

I have had this service dog for 3 years now and take him everywhere I go.
