Question: I need to know about leprosy .If a person takes multidrug therapy for one year .is there any chance of getting reactions after treatment ?what is downgrading lerpa reaction ?

Thanks for the query.
Leprosy is a very slowly progressing disease and the progression of disease totally depends on immunity.
Multi drug therapy for 01 year reduces the bacterial load significantly but some of the bacteria remain latent and get reactivated when immunity is low this is called downgrading reaction.
If a person is having downgrading reaction then medicine should be continued and focus should be on improvement in diet.

which medicine need to continue...other doctors told that treatment is completed ,no need to take any medicine .As he takes MDT for one year and prednisolone for 3 months. Now that patient is fine but only thing is he feels pain in eyes and water continuously coming out from eyes.He is feeling muscle weakness ,what he need to do in order to strenthen muscles.

he is unable to eat with his hand ,is there any possibility to lead normal life after hand surgery?

Thanks for the follow up.
MDT needs to be continued if there is downgrading reaction, that is if new patches of leprosy have appeared or if there is fresh weakness of muscles.
If no such changes are there then MDT can be stopped and supportive therapy needs to be started.
Start the following.
1. Tablet methylcobalamine 1500 microgram daily in morning
2. Tablet Iron daily one
3. Tablet calcium daily one
4. carboxy methyl cellulose eye drop three times a day.
Continue medicine for 6 months.
Regular physiotherapy of affected hand is must to regain strength.
Usually there is some improvement in muscle power with time, it might take 2 to 3 yrs for improvement.

Reappearance of patches means that the disease is still active and MDT is still required.
MDT is safe and can be continued till there is complete resolution of disease.
Leprosy mainly affects nerves hence the damaged nerves cause burning sensation.
Tablet methylcobalamine 1500 microgram daily can help in faster healing of nerves and relief from symptoms.
