Hello XXXXXXX
Welcome to Ask a Doctor service!
I passed carefully through your test reports and would explain that these cholesterol levels are not concerning.
They are borderline, which means that they are mildly elevated. You should know that LDL values are not the best indicator of dyslipidemia.
We usually make our therapeutic judgements based on Total cholesterol/HDL ratio (which is 4.8 in your case, while normal values are below 4.5) and LDL/HDL ratio (which is 3.3 in your case with normal values below 3.5).
So, the situation is not so serious. You should know the glucose metabolic impairment plays an important role in these high cholesterol levels too.
Another important issue to consider is your thyroid gland function, as thyroid gland dysfunction can trigger dyslipidemia.
In my opinion, even with diet modifications we can achieve normal cholesterol levels. In this regard I would recommend avoiding fat and sugar food (including deserts, even bread and pasta), fast food and try to eat more vegetables and fibers (including cereals). Mediterranean diet is recommendable, including some changes for diabetes patients (avoid sugar food and try to eat more frequently an in small portions).
Aerobics can help reduce LDL values too. If you are overweight, I would recommend trying to loose some pounds and this would help improve your blood lipid profile too.
If all these natural tips don't help, I would recommend taking for a month or two a statin (atorvastatin) at a low dose and see if your situation improves, when taking this drug coupled with the above mentioned modifications in your lifestyle.
I don't think that you will need statins for all your life, if you follow a careful diet, a healthy lifestyle and try to maintain normal blood glucose levels.
Besides, considering the borderline cholesterol levels, there is no reason to panic about a heart attack.
Anyway, I would recommend periodic follow up with a resting ECG and a Doppler ultrasound of the carotid arteries (in order to investigate for atherosclerotic plaques) every 12 months.
The blood lipid profile, coupled with HbA1C should be repeated every 3 months.
Hope to have been helpful!
I remain at your disposal in case of further questions whenever you need!
Kind regards,
Dr. Ilir Sharka, Cardiologist

Dr. Sharka,
You have been so helpful and prompt with your response. I truly appreciate you more than you can imagine. A little bit more information about me. I started on October 26, 2020 weighing 179 pounds. I began on that day treadmill exercising of 1.0 mile a day at the speed of 2.0 and my speed changes occasionally from 2.0 to about 3.4 speed. There are a few days that I go farther as far as 2 mile on the treadmill. Presently today, I am weighing 163.4 pounds but my blood work is not changing but much at all. I would have thought that by losing already 16 pounds my blood work would have been considerably better but not the case. I'm becoming frustrated because I don't know what else to do... I uploaded two more reports from Oct and Nov of 2020 for comparison of blood test results now. From comparing them, what do you think is going on or what should I be doing differently to achieve better results? Is there any type of bread or pasta that would be safe for me to eat? Can I even eat a steak once a week or two or never? I really do miss my rib eye steak... I just want to be healthy and lose weight, that's my ultimate goal.... Thanks!

Dr. Sharka,
I forgot to ask you would it hurt me or possibly help me to do a LIVER DETOX? Even though my AST and ALT came back within normal range, could this still help in the process of helping my cholesterol and lowering my weight? I read a lot of reviews that spoke highly of doing this.....
VitaLiver is the name of the product I would like to try: I would take 1/4 teaspoon (1-3 times a day as advised by the doctor). Below are the ingredients in the VitaLiver product. Please keep in mind I take Losartan HCTZ, Bystolic, Synthyroid and Norvasc once a day.
VitaLiver Liver-Health Cleanse and Detox Supplement with Milk Thistle - All-Natural Liquid for 2X Absorption - Chanca Piedra, Dandelion, Artichoke and More
I truly just want better health and to get in shape.....
Thank you!

Hello again!
I carefully reviewed your blood lipid profile in the previous months and would explain that the results are very similar, with very small insignificant changes.
Coming to this point, I would recommend trying aerobics and some herbal supplements which can help lower your cholesterol levels naturally :
- garlic
- ginger
- soy (including soy bread and soy pasta)
- vitamin B3 (niacin)
- psyllium (this is a herbal supplements against constipation, but which can help lower cholesterol levels too).
If the above diet modifications don't help, I would recommend taking low dose of atorvastatin.
Hope to have been helpful to you!
If you have any other questions, please don't hesitate to ask me again!
Wishing all the best,

Doc, I have no problem with trying these supplements but how much of each one do I use daily? Also, can I use the kyolic shed garlic with lecithin? I heard some negative things about the product but my sister had it already so I would just use hers or should I just use plain garlic capsules and if so, how much and how often? Also, are you saying for me not to use the liver detox supplement? I need to be perfectly clear about things. Please explain clearly.
Thanks!

Hello again!
I would recommend taking the liver detox supplement and also the garlic capsules.
You can take garlic capsules twice daily if they don't lower your blood pressure too much.
You can take ginger, 60gram per day, cut it into small pieces boil it with 1liter water and drink it, maybe with some lime and parsley inside. Parsley and lime do not need to be boiled, but you can put them inside the ginger water, during the cooling process and drink this type of drink as a lemonade during the day.
Hope you will find this information helpful!

Hello again!
I understand your concern and would explain as follows:
- garlic capsules can be taken 1 cap per day and increase the dose up to 2 capsules per day depending on your blood pressure values.
Another way to avoid capsules is to put garlic in your food, by cooking.
- there are different types of supplements in different quantities of vitamin b3 and psyllium. It is ok to take small doses, based on the instructions on the bottle.
There are no interactions between these drugs and liver detox.
Regarding cereals, you can take oatmeal, which is really good. A first step to lowering your cholesterol is having a bowl of oatmeal or cold oat-based cereal like Cheerios for breakfast. It gives you 1 to 2 grams of soluble fiber.
Regarding soy bread and pasta, you can find them on gluten free products in the supermarket.
Regarding the steak, it is ok to eat one once a week, with plenty of salads during the week. It is also important avoiding butter and taking olive oil.
Fish and fish oil is also good to help increase HDL levels, which is the good cholesterol.
Hope you will find this information helpful!
