I Recently Did A Brain Mri Which Showed Sphenoethmoidal Recess
Question:
I recently did a brain mri which showed sphenoethmoidal recess retention cyst 1.5cm! The reason I had an mri is because I felt dizzy, nauseous, and my eyes felt weird. My questions are, is this cyst new? I%E2%80%99ve never had any sinus infection! But in the last week I%E2%80%99ve been coughing a lot. Also, is the cyst in the ethmoid or sphenoid sinus? Would you recommend I remove this, my ENT said he could do it no problem. I feel between my eyes also that%E2%80%99s bothering me.

Hi,
Thank you for your query.
1. If you share your MRI images, I will be able to guide you with greater accuracy. If any surgical intervention is planned an CT PNS (Paranasal Sinuses) Scan is necessary in addition.
2. Cysts in these regions are mostly mucus retention cysts. They are often discovered incidentally. It may have been there for years or developed recently as you have a cough. There is really no way of knowing as there is no previous imaging to compare with. It may have arisen or increased in size when your symptoms appeared.
3. If asymptomatic, they may be left alone and observed at regular intervals.
4. In your case, as you are feeling dizzy, nauseous and have discomfort in the eyes, it is necessary to review the scan images.
5. Only spheno-ethmoidal cysts are rare. However cysts arising from the sinuses are more common. Such cysts may arise from the sphenoid and ethmoid sinuses.
6. Cysts are usually marsupialised (opened up) or excised depending upon the exact anatomy. Hence a review of your scans and sometimes a diagnostic nasal endoscopy will help.
7. Rarely these cysts may be some other type of lesion such as polyps and many other rarer conditions. Viewing the scan images (both CT and MRI) will help differentiate these conditions from cysts.
I hope that I have answered your query. If you have any further questions, I will be available to answer them.
Regards.

Need more images around the third image.
Detailed Answer:
Hi,
Thank you for following up
1. I need to see more images around the third image.
2. Preferably, share all the images.
3. There is a small lesion in the leftsphenoethmoid area not involving the neighbouring sinuses.
4. This may turn out to be a fungal infection such as aspergillosis.
5. Here the CT Scan PNS images will help immensely.
I hope that I have answered your query. If you have any further questions, I will be available to answer them.

Brief Answer:
Plain CT PNS (Paranasal Sinuses) is necessary.
Detailed Answer:
Hi,
Thank you for your query.
1. A plain CT PNS is now necessary to check thr bone partitions between this cyst/polyp and the optic nerves, sinus walls.
2 From the limited MRI images provided, the retention cyst is seen arising from the left sphenoid sinus and exiting through the natural sphenoid opening into the sphenoethmoid recess. The sphenoid sinus opening does not appear to be enlarged by this cyst/polyp.
3. Bone is invisible on MRI Scans but visible on CT Scans.
4. It is possible that the left sphenoid sinus is smaller than the right sphenoid sinus which is common.
5. Coronal, Axial and Sagittal CT Scan images are required to plan further management.
I hope that I have answered your query. If you have any further questions, I will be available to answer them.
