Brief Answer:
choking and near vomiting is indeed needed to be assessed at the earliest..

Detailed Answer:
Hello and Welcome to 'Ask A Doctor' service...
I've gone through your query in details...
I can understand your concern...

Well, let me come directly to your queries...if the cough is severe and causing such choking episodes repetitively, then you shouldn't delay any longer and just refer to your nearest ER for a prompt assessment...

The answer to your second question (pertaining to Goiter) is both yes and no. No, if it's not significantly enlarged(can be assessed exactly only after an Ultrasound of the neck). However, it can certainly act as an aggravating factor given the upper respiratory tract is already inflamed (due to either some allergic manifestation or infection) which is also causing so much of cough.

Additionally, please refrain from having any cold beverages, extreme sour foods as they can further worsen the symptoms. Avoid going to any dusty/overcrowded places for the time being.

Always ensure to wash your hands with antibacterial hand-wash before and after handling the respiratory secretions, sputum etc.

Start taking steam inhalation three times daily, do some lukewarm saline water gargling, it'll bring some symptomatic relief.

29th is indeed too long, at this moment, I wouldn't recommend any other over the counter remedies as they can further mask this situation, so please be advised to the ER/your Doctor for a prompt check-up. Further investigations may also be needed to be done to look into it further.

Have plenty of hot beverages (soups, broths, hot menta tea) as they'll work as an immunity booster.

Let me know if I can assist you further.

Take Care
Kind Regards
