Ever since my surgery, I have been unable to ejaculate semen. Surgery was 4 years ago.
Two years ago, I started to have pain before and during an orgasm, which immediately went away after. This pain radiates across my pelvis in between navel and pubic hair. The pain sometimes hit my lower back.
What can be wrong? I recently had an MRI (I uploaded the results), and it states my seminal vesticles are vertical.
What does that mean?
Is my seminal vesicles in this position prevents semen from coming out?
Is this the cause of the pain?
Does prostatis cause this type of pain and lack of ejaculate fluid?

I would suspect that pelvic autonomic nerves located at your seminal vesicles were damaged during your proctocolectomy. This would cause either absent or retrograde ejaculation and possibly also a form of prostatitis. To determine if you have prostatitis, a rectal exam is necessary. It can typically be treated with long term antibiotics but the absent ejaculation is unfortunately not treatable.
