Primary hypertension or hypertension that comes with age may come suddenly and may not be related to any secondary cause and still be very high Bp
So, this argument that BP reading is very high does not hold true for secondary hypertension.
But still it can be secondary hypertension.
causes
1. renal parenchymal- reports are normal but we dont have ultrasound report with kidney sizes.
2. reno vascular- This can be there as your age is > 60 and there is history of tobacco use. this is mainly because of atherosclerotic vascular disease involving renal vessels- here also need ultrasound report initially
3. Endocrinal - cannot comment will need detailed history.
Now BP should be controlled with anti hypertensive agents you are only on one medication.
You have to increase the doses and number of medication under guidence of your doctor. consider ARBs .
Dont worry blood pressure will come down. Try not to take foodstuffs rich in salt. decrease your salt intake. can take diuretics for swelling will also help in BP management.
weight loss and significant anemia-- i am not clear on the diagnosis with the information provided but if there is further weight loss and anemia persists it should be investigated in detail.
