What causes lactic acidosis on when someone is on metformin ? Also why do people start on 500 mg 2x a day is it dangerous to start on 1000mg 2x a day

Brief Answer:
The Initial dose of Metfmin is 850 mg orally once a day.

Detailed Answer:
Hi
I have gone through your query and your attached reports.

Your HbA1C result is high and this means that you can't control and manage your diabetes without medication.

Considering your last Fasting blood sugar and your HbA1c result i suggest to start Metformin 850 mg ,1 tablet after dinner and 1 tb Gliclazide MR 60 mg before breakfast. The dosis of each drug can be modified accordingly to your blood sugar result in the future.
At the same time it is very important to follow the right low carb diet and to exercise (20-30min)/every day.

My advice is to consult physically your local endocrinologist for the prescription and the follow up.

Lactic acidosis happens in patients with high dosis of metformine and is often seen with accumulation due to kidney failure, liver failure or overdose.

Hope this is helpful.
Take care

Can lactic acidosis happen at the doses you mentioned ? Also is it ever possible to reverse diabetes so you don’t need medication forever? I’m obese and trying to lose weight if I lose it can I put the diabetes into remission?

First :-Plan weight loss goals!

Detailed Answer:
Hi again

No,lactic acidosis don't happens with 850mg/day metformin. You have no reason to worry about!

If you lose weight and you follow a healthy lifestyle with daily physical activities and a strict low carb diet you can have normal blood sugar numbers without medication.
But for the moment you need the help of medication to lower your HbA1C and blood sugar results.

I suggest to consult a dietologist for a specific hypocaloric hypoglucidic diet, to exercie every day and to stay motivated because losingweight is the first step to manage your diabetes.

Hope this is helpful.
Wish you all the best
