I Ve Been Diagnosed With A Likely Mid Anterior TZ

Question: I've been diagnosed with a likely mid anterior TZ csPC 2.2 cm widest diameter. Does this translate to a roughly 8 cc lesion? Is this likely fatal? It is adjacent to the capsule on mri. THX!

Thanks for the query.

Based on the facts and gross report of MRI that you have posted you seem to have Localized cancer of the Prostate. However, it is a routine protocol to do MP MRI guided Trus Biopsy of the Prostate to decide the grade of the prostate cancer and further modality of treatment.

Since it is localized cancer and has confined to the prostate gland and not infiltrated beyond capsule I can state that it is not fatal at all and can be treated either by Radical Prostatectomy or Active Surveillance.

Thank you. The mri description is;left apical mid anterior transition zone lesion.
2.2 widest diameter
t2w score=3
dw mri score=5.
dce mri score=positive.
overall prads score=4/5.
NIH score=moderate.
I'm assuming this is clinically significant cancer with a 75% probability based on the prads score. I have low PSA(1.6) which is worrisome as well as the size of the lesion.
The radiologist estimated epe to be approx 40% so I'm guessing my risk of metatesis to be approximately .75 x .4=30%. Would you have more realistic numbers I could use? Thank you so much. You've been very kind to me and I appreciate your candor and concern. XXXXXXX

As of now there is no evidence to suggest possibility of Metastasis

Thanks for follow up .

As per Radiology report cancer is confined to prostate glans .As regards possibility of future spread and metastasis can not be predicted in any cancer .However prostate cancer is very slow growing cancer .

The only criteria and confirmative to predict future progress of the disease and modality of treatment is decided only on the outcome of prostate Biopsy report in terms of Gleason's score .

But in view of the present situation either Radical Prostatectomy or Active Surveillance is the best option .

I assume my lesion is larger than most @2.2 cm? Is my low psa(1.66) a concern? My prostrate volume is 33cc. Which biopsy(i.e. in bore mri-targeted) is considered the best? Which institutions have the best reputation for dealing with prostrate cancer? Are csPC's slow growing? Thanks again.

Size of the prostate cancer has no relevance as far as prognosis is concerned .

Raised PSA (Above 4 )is considered to be a primary tool to suspect prostate cancer and demands further tests like Ultrasound Scan ,Trans Rectal Ultrasound ,MRI to confirm the diagnosis of cancer of prostate However low PSA does not rule out cancer of prostate.

MRI targeted Prostate Biopsy is the most reliable test to confirm the grade of the Prostate Cancer.

In U.S there are multiple centers almost in every state that are equally advanced in diagnosis and management of cancer of Prostate XXXXXXX would be nearest to you .

Prostate cancer is as such very slow growing cancer compared to cancers of other organs in body.
