Question: I want to ask about recent encounter I had two time massage service ended up with hand job and Brest job using tits on the August 2nd time she was completely naked body to body massage and I have not done sex, the massage lady have touched my genitals a lot with different part of her body except her mouth and gentiles. She were fully naked setting on my back doing the massage and i got in contact heavily with her body upper part like nipple chest tits was too much I had lots of friction with her body , she also heavily kissed and licked my nipple a lot .. since that night , 2/3 days after the 2nd massage later I developed a strange skin rash on my upper thigh, chest and legs only that lasted for less than 3 days and then I had serious fatigue with fever of 38.7 c lasted for 3-4 days and gone with ibuprofen nowadays I have pain in the lymph near my ear and dhairia & most of the time I just want to sleep and do nothing like fatigue and so tried. Note: she was completely naked and I can feel her gentals sometimes. I have gone to testing center on the 9th August to test for hiv combo week after the incident and found negative also again 16th combo test nigative and on 22 tested something called HIV antibody test and took 20 min and found negative. My questions are:
1- are these test accurate ? Only 10-15 mins ? They look like pregnancies test?
2- what is the risk of this encounter? And should I just forget and proceed normally with my wife? 3- when should I get tested more? If I need?
4- based on googling on the past week I see majority are saying the hiv don’t transmit via contact skin to skin but how would I know if she may have some minor scar and I do have many pimple on my back , or micro scar?
5- I have plenty of dead skin from pimples in my back and some are new ones, is their a chance that it can be source of transmission?
6- what is the chance of syphilis and other STI?
7- when I should get tested for syphils for a confirmed results?
8- should I presume my unprotected sexual with my regular partner?

Hello and welcome to ask a doctor service. I have gone through your query and noted your concern.

There is no risk of HIV through skin to skin contact as in handjob or breast job or through kissing and nipple licking. HIV is only transmitted through either vaginal, anal or oral sex. There has to be either direct contact with infected genital mucosa or exchange of infected genital secretions for HIV to pass through.
Minor scratches or pimples on skin do not provide access to the virus so you don't need to worry about that. Moreover sweat is not an infectious fluid..
As far as HIV testing is concerned, there is a waiting period of 4 weeks for the test to be considered reliable but in my view you don't even need to test for HIV as there was no risk of it from the kind of contact you had.
The tests that screen for HIV is a rapid test and detects circulating HIV antibodies against the antigens in the testing kits. It is a bit similar to pregnancy test kit.
Syphilis too is usually transmitted sexually. Kissing and licking nipples are uncommon routes unless she had syphilitic ulcers or sores in her mouth.
There is small risk of genital HPV and genital herpes if she rubbed her genital skin against your genitals.

today I have completed 29 days since that exposure so I tested quick fourth gen HIV and syphilis test but I don’t know the type of the test it was through a blood sample. Both results were found negative .. the last follow up would be :
1- I don’t need any further STD testing for assurance?

2- Recently I noticed some testis pains that comes without any direct hit but have the same feeling, they are not continuous pain but twice a day if I walked ,is it related to any STI any recommendation ?

3- what is the name of the sypilis blood test and is it accurate at 29 days?

Detailed Answer:
Hi.
You certainly don't need any more HIV screening test. A 4th generation HIV screening test is the latest one that is an ELISA TEST. It is conclusive at 4 weeks. A VDRL test is the screening test for Syphilis. It should be done at 4 weeks and then again at 12 weeks for a conclusive result though most of those infected will test positive at 4 weeks from exposure. I don't think you need to worry too much about it but go through the test at 12 weeks. I don't think transient testicular pain has anything to do with STDs considering the nature of exposure which is non risky.
