Question: I was diagnosed with post viral poly arthritis, however, the diagnosis does not seem complete. I have swollen knee and pain which makes it difficult to walk. I was hospitalized for 4 days when I could not walk and had fever. I had CRP of 16 mg/L on 7th Oct. CRP on 23 Oct was 30.30 mg/L. This was after discharge from hospital. I have ESR of 81 mm. Tests ANA, Uric Acid, RA, Chikungunya IgM, Dengue, CCP Antibody, WIDAL, Malaria, Procalcitonin, ASO, Typhi are all normal or negative. Need your help and advise on next steps. Hi I am Dr Gopal Goel Orthopedic Surgeon.

I have read the details and of particular concern is increasing CRP- Indicating rheumatoid like pathology.

Post viral Arthritis is very common and more so in today's time with increasing virulent infections.
Your history reveals that you have fleeting arthritis- that is inflammation of one joint and when it goes off or reduces ,other joint may be involved. This can be a feature of post viral arthritis.

I would treat you as a case of Rheumatoid but decrease and stop medicines gradually as the symptoms subside which may take few months. I would stop steroids and start Disease modifying agents like methotrexate .
