Brief Answer:
soon after ingestion usually

Detailed Answer:
Hello,

stomach pain may occur soon after ingestion or a few hours later. I wouldn't expect stomach pain to occur days later. Please keep in mind that azithromycin may also cause intestinal problems; antibiotic-related diarrhea or even pseudomembranous colitis are also possible and they may cause abdominal discomfort or pain in various locations including the upper abdomen. Intestinal problems may occur right from the start but they may also occur many days after starting the antibiotic.

I hope you find my answer helpful. Please let me know if you need further assistance.
