Question: I am 63 year old man and was diagnosed with BPH have been seeing a urologist for a few years last PSA was 1.42 about 18 months ago, am taking flomax and symtoms are about the same the urgency to pee sometimes comes on quick and still go about 2 times at night. I work on my feet as a chef and my back and hips are hurting about 4 hours into my shift, and this has been going on for a few years CT did say degenerative changes in spine and hips had progressed some

I have gone through your query and here is my advice.

As per your history, you have BPH and now having same symptoms urgency to pee.

Please let me know,
Do you have burning while urination, fever, pain in abdomen?

For that I would suggest you to consult with urologist for detailed clinical examination. He may procure complete hemogram, urine analysis, urine flow study, ultrasound abdomen sos as per evaluation to rule out urinary tract infection/ increased prostate size.

For degenerative changes in spine, I would suggest you to repeat MRI back after spine surgeon consultation. Further treatment mainly depends upon the underlying disease severity.

Till then for pain you can take tablet ibuprofen 400 mg. Appy diclofenac gel locally. Avoid forward bending. Use lumbar belt. Continue physiotherapy.

I had 2 ct since that MRI, showed some spinal stenosis more advanced hip arthritis and changes. I had 3 bone marrow biopsies for that 11 years ago . I am okay in morning but like now am I am having hard time straightening up when I bend over which I do a lot when I touch my chin to chest pain in right side front near shoulder

Thank you for your reply.

As per your history, as you don't have burning while urination so it may not be urinary tract infection. So kindly consult with urologist and get done urine flow study and ultrasound.

If possible,kindly upload bone marrow biopsies report. As you are having stiffness specifically in the morning, so kindly consult with rheumatologist. He may procure RA factor, c reactive protein, ESR to rule out rheumatoid arthritis.

For spinal stenosis, kindly follow all measures as suggested earlier.
