Question: I would appreciate your advice on a dilemma I am having with my testosterone replacement therapy. I am on 120mg/week of testosterone cypionate.
On my “trough” day my level of estradiol was 155 (normal range is anything less than 162). Please note that I did not have any estrogen symptoms, I was just trying to optimise because if my trough day is high, it seems to me the other days would be even higher, so I decided to try 60 mg/every four days. There is a lot of online discussion supporting this practice of increasing the frequency but lowering the dose in order to reduce estrogen conversion.
After 8 weeks on this pattern my trough day estrodial was 151 (normal range is less than 162.), which is not much different than the once/week regimen but I understand that estrodial measurement is not as precise as testosterone? Do you think the twice a week regimen is worth it?

Hope this response fing you well.
I read our previous correspondence ,so your history with hormone replacementy therapy is clear for me ,now.

The first think that i want to underline is to not take decisions,for your medications,based on "On line discussions".

As i daid earlier,Hormone replacement therapy is a complex process and requires careful attention to symptoms, as well as laboratory testing.
For this reason the follow up process can be done only by a specialist(an endocrinologist or a hormone replacement therapy specialist)

The dosage is decided of testosteron based on your needs.
In general, the recommended dosage is 50–400 mg injected into your muscle every 2–4 weeks.
Your doctor may adjust your dosage based on your testosterone blood levels, response to treatment, and side effects.

The problem of T therapy is that some of the testosteron is converted to estrogen from the aromataze enzyme.(we have discussed about this topic,in the past)

It is not proved that increasing the frequency of testosteron injections,reduce estrogen conversion.
At the other side the estrogen test result with "the new regymen" is the same with once a week regymen.
Considering this results ,it is evident that twice a week regimen is not worth it.

You do not have "high estrogen" symptoms after injectable T therapy and your actual estrogen level is still within the range.
Considering this,i think that the regymen with 120mg/week of testosterone cypionate,is OK for you.

If you feel fine with this dosis ,the only advice that i have for you is to repeat the necessary tests every 6-12 months(the estradiol ,testosteron total and free measurements , PSA test, CBC and Comprehensive Metabolic Panel w/EGFR) to monitor the possibel side effects of the therapy.
