Question: Hello. I would greatly appreciate your input. I began treatment for scabies via ivermectin, permetheron and several antibiotic ointments almost four weeks ago. The itch has improved but the painful lesions show no signs of healing. In fact, I am noticing new burrows and lesions with each passing day. I've requested another dosage of ivermectin but my doctor is unresponsive. I contacted her office this morning and was assured that someone would contact me regarding my concern. Unfortunately, there was no contact. I'm also concurrently going through an intensive cycle of ECT for my treatment resistant depression. As you might imagine; an infestation of scabies is doing very little to improve my state of mind and anxiety . I'm 59 years old and feel as if there isn't some improvement with my skin soon, I can't imagine how I'm going to carry on.....More specifically, if I want to carry on. I've had other skin problems through the years which doctors were quick to blame on my depression.....accusing me of self infliction of the sores. I'm 59 years old was diagnosed with major depression at age 18 and there is no record of any self mutilation I might have engaged in. That has never been a part of my story. Is there any way to get the attention of my dermatologist and have her proceed with the very most aggressive treatment? I realize that permetheron and ivermectin are first line treatments but it appears as though I may need multiple applications along with any other treatment that might be beneficial. Some nights , I don't feel like I can physically or mentally handle this.....but there is nowhere to go. In addition to all I've described, my vision has also been impaired. Blurriness and constant stinging. I thankyou for taking the time to read this and would appreciate any input;

Hello,

I am sure you will get the required help from your doctors.
You can try 3-4 rounds of permethrin with a gap of 2 weeks. If that doesnot help then Lindane lotion can be tried instead. Since it is poisonous, it is used carefully. You can read it precautions here: https://www.accessdata.fda.gov/drugsatfda_docs/label/2018/LindaneLotionMedGuide.pdf
Lindane should be applied lightly over affected areas, neck down keep it on overnight (max 8 hrs), in morning scrub the burrows to remove the parasite, then rinse it off nicely with soap and water.
There should not be issue for getting prescription for another dose of ivermectin, if needed.
Eyes need to be evaluated by an eye doctor urgently as scabies is known to cause vision changes and even blindness.
