I am so sorry that I had overlooked your last response.
I have checked the attachment. The salient points are:
* Blood sugar is raised indicating diabetes or poor control of diabetes. Blood HbA1c and Urinary ketone bodies and sugar indicate the same.
* Liver enzymes are raised. This can have an association with diabetes.
* Sodium level is low. Extra salt in the diet is advisable.
* Lipid profile is grossly abnormal indicating requirement of urgent intervention.
* Other findings are not concerning.

I thank you for going over my blood work but I was mostly concerned with my sodium and hematocrit levels. You told me I am in "pretty bad shape" which I though was in appropriate. Since my last blood work everything has gone down. Lipid panels might still be high but on a downwards trend without medication. My blood sugar is going down as well. So with that being said, is my sodium and hematocrit level a big problem?

Regarding the hematocrit, I would not be worried. Regarding the sodium levels, some oral supplementation with extra salt should suffice.
