Question: I would like to know if isotretinoin is metabolized via CYP450? If I am a poor metabolizer will this medicine be okay for me to take? How often/what percent of the severe side effects occur?

I have reviewed your query and here is my advice.

I will suggest to try all other options including systemic antibiotics before trying isotretinoin especially in young age.

Skin and mucous membrane related side effects occur in almost all patients taking this medicine. Hypersensitivity reactions, pancreatitis, hepatotoxicity, hearing impairment, inflammatory bowel disease may happen. It may also affect the bones, spine and the lipid profile.

After absorption, it produces 3 metabolites. There are some studies to suggest that it gets metabolised by CYP450. One of the metabolites is suggested to be more active than the parent compound. So, if you are poor metaboliser then the side effects will be more.
