I'm A 19 Year Old Boy. It Has Been Happening
Question: I'm a 19 year old boy. It has been happening from about 1 year since now, every week I have a kind of sensation in the soft palate inside my mouth. And after some time, some white substance comes out of it...which appears like a mushed up worm, as shown in the annexed images. What is it? Should I be concerned? I haven't consulted a doctor before because I don't know what kind of doctor would help me with this issue. Please help!

As per your symptoms it seems like a post nasal drip which happens in chronic sinusitis. However it looks like thick sputum not a worm like thing.

The thing looks like tonsilolith or tonsilar stones. These are nothing but a stone formation due to chronic inflammation. Mucus plug is another possibility.
However we can not confirm just by looking at it. You need to send the sample for biopsy for tissue diagnosis and a biopsy can exactly tell what it is. I suggest you to send the sample to your nearest pathology lab for confirmation.
It is not a big deal unless you are symptomatic.
