Question: In 2012 I was diagnosed with CAEBV (Numbers exploding). Can children be infected at birth? Also, according to my cardiologist I was infected at a very young age, and he doesn't know but I was raped repeatedly for a number of years ever since I was lil and I think that's how I got infected. Is there any way to proof how and by whom I was infected?

Hello!

Welcome to - Ask a Doctor - service!

I passed carefully through your concern and would like to explain that you are right about the possibility of being infected through sexual intercourse.

Anyway it is difficult to confirm the precise time of infection; and this is due to several reasons;

- EBV may be present with the human body without being active from the beginning;
- individuals rarely perform yearly medical check ups through blood tests to confirm they are free from this virus infection;
- even they are in front of situations with high probability of transmission lab tests are rarely performed.
- EBV is not transmitted only by sexual intercourse, but also by using infected objects like a toothbrush, or a contaminated drinking glass, by saliva, etc.

Regarding the vertical transmission (from mother to child during birth) there exist a possibility of getting the infection; anyway it is not high and it is difficult to predict. Tests of PCR-viral DNA can be done to the newborn to clarify whether it has been infected or no.

You should discuss with your doctor on the above mentioned issues to find the best way of managing and preventing infection transmission and treatment.

Hope to have been helpful to you!

Due to the nature of my concern I would appreciate a more serious and considered response.
I am Aware how rarely this disease is tested for, I underwent many tests for years before a doctor finally thought to test for CAEBV.
I do not need a precise date of infection, but I do need to narrow down the age range. I have already established it occurred at a young age through my previous doctor as well as the fact that I have had issues with my body "Behaving as though it is immunocompromised" without knowing why, also since a young age, and doctors being unable to discover the cause.
I am interested in tracing the source of my T-Cell CAEBV so I would like you to please address testing options for viral strain and type which can be traced to the person who infected me since I am confident this can be narrowed down to 2 people, my so called parents. Neither of whom would have "shared a drink" to save my life.
I do not currently have the option to discuss this with my doctor and that is why I am asking here. Also, I want to make sure I cover all bases due to the rare and little understood nature of T-Cell CAEBV.

Thank you!

Brief Answer:
My opinion as follows:

Detailed Answer:
Hello again!

Let me explain that an approximate period of infection (whether a long time ago, several years of just a few weeks could be determined by checking various serologic responses in EBV diseases.

The underlying tests include EBV antibody responses; anti-VCA (IgM Monospot/Heterophile, IgM, IgG) and anti-EA (diffuse EA, restrict EA). By controlling the above responses combinations it can be defined an approximate period of infection.

Further serologic specification would need complicated DNA testing by advanced techniques in specialized laboratories. But if you restrict the number of possible transmitters, then it would be easy to arrive to an approximate conclusion, by checking their blood and defining which results positive. But that is not enough, as positive false tests may result from concomitant infections as well (like HHV-6, toxoplasmosis, CMV, rubella, anicteric viral hepatitis, etc.)

If a medical center with laboratory expertise in detecting infectious disease is available at your living area, you may ask for the above mentioned tests and investigate further the issues of your concern.

Wishing you good health!

Regards,

Dr. Iliri
