Question: In January of 2020 I was diagnosed with Epstein Barr Virus after positive PCR tests, correlating symptoms, and findings of splenomegaly on an abdominal CT scan. After a few months of fatigue, my infection serologically resolved and the virus was cleaned from my system. I recently had an abdominal CT for an unrelated condition and found that my spleen was still enlarged at 15cm. Blood tests were done to check for infection and cancer markers; everything came back normal (including blood cytometry) except a moderately elevated white cell count. My splenomegaly has been assumed idiopathic and I was told to get an ultrasound in 3 months for a follow up. Is it completely out of the realm of possibility that my spleen is still enlarged from the previous EBV infection? Can splenomegaly remain even after infection has resolved? I'm 31/m and very concerned something malignant is developing. Thank you!

Hello and welcome to Ask A Doctor service.
I have read your query and here is my advise.

* As per my clinical experience, the spleenomegly take varying time in
months depending upon the individual immunological response to revert
back to normal previous phyiological size and at times, that do not go
back to completely normal size also, but do not pause any harm lifetime.

* As the serology markers have reverted back to normal, the infection is well
controlled.

* Though there are wide variations in parameters for spleen size in different
sex as well ,the normal size of adult spleen is average of 14 cms and that
in taller individual is 15 cms.

* However , if you can provide me previous ultrasound size of spleen , that
would be better to co relate in your case.

* By the way, cancer is of no concern at this point of time as malignant
spleen is far beyond 20 cms in usual instances.
