Question: In addition to lightheadedness, I have experienced the other symptoms listed: slight-momentary nausea, went to sleep last night at 9:30 and never go to sleep before midnight (sleeping disorder), wobbled badly last evening and all day today. but better now. Have stopped and started various meds Sunday. Checked BP this afternoon and it was very low for me, 85/45 (currently 81/44), and is usually around 125/85 , pulse went up from about 70 to 80 (currently 74, taking Plavix and Lisinopril for hypertension). Have mild type 2 Diabetes, ischemic heart disease (5 bypasses 9 years ago and 2 stents since), moderate GERD and acid reflux, severe restless legs syndrome and REM behavior (just changed to Horizant and Baclofen); taking usual meds for all these issues., among others (15 Rx and 5 OTC). Played golf in Florida heat yesterday and played bocce ball last evening. Wife thinks that I might have had dehydration, but I have played golf many times in hotter weather than today all Summer. This noon I took all the drugs that I missed yesterday evening and night and this morning, plus regular noon drugs -- too many at once -- I am concerned about drug interactiion, but lightheadedness began before today when I took so many meds. Lightheadedness was so bad last night that I was not only wobbley last night and this morning, but I also had vision problems -- couldn't read anything that I normally can read. Issues are so complicated that I might have had one problem last evening and another today. Sorry for the excessive issues, but I am most worried about my lightheadedness. What do you think is causing all these symptoms, especially lightheadedness and unstable walking?

I have reviewed your query and here is my advice.

You seem to having side effects of Baclofen. It is a muscle relaxant and can cause lightheadedness and wobbly walk. You are not supposed to operate heavy machinery or drive when you are on Baclofen.

If these symptoms started after starting Baclofen, then the cause is confirmed and we need to be more careful. Maybe reduce the dose or withdraw as per your doctor's advice.

If symptoms started before Baclofen, then kindly send me details of your symptoms - when it started, how it started and which medicines you took at which time.

Take lime juice with salt in it to manage your lightheadedness. Do not make sudden movements. Better to stay indoors with help around.

Thanks Dr. Punj for opinion. Went to hospital emergency shortly after asking this question. They admitted me to hospital concerned about possible stroke or TIA. They determined that I did not have stroke and, after brain MRI, that I probably didn't have TIA. My cardiologist checked me while in hospital and determined that it was probably not my heart. They discharged me with conclusion that I "possibly had TIA." I will see my primary care physician next Tuesday and will share your thoughts about Baclofen, and will stop taking Baclofen until then. Your thoughts about Baclofen are credible, especially because my symptoms have not entirely gone away since leaving hospital, especially mild unstable walk and mildly confused behavior, and because I started taking 10mg Baclofen 4 times daily cold turkey. Of course, it could be something else entirely, like a TIA, or dehydration, or heat stroke. What do you think about the possibility of TIA, dehydration, or heat stroke? Thanks, Tom Fulks YYYY@YYYY

Regarding TIA, your symptoms are somewhat suggestive of brain issue, but stroke is mostly one sided paralysis and TIA is temporary symptoms (similar to yours) lasting less than 1 day.

Similarly sun stroke or dehydration would not last long (not more than few days) unless there is low sodium that has affected the cerebellum. We can check the serum electrolytes (especially sodium) levels in both cases. Sun stroke will also have a high body temperature after exposure.

Dose of Baclofen is as follows:
Initially, 15 mg daily in divided doses; increase gradually according to response. Suggested regimen: initially 5 mg three times a day for 3 days, can be increased to 10 mg three times a day for 3 days, then similar increments until either a dose of 20 mg three times a day is reached or until the desired clinical response in patient is obtained. When side effects appear, dose may be lowered or stopped.
It is to be withdrawn gradually if there is no response within 6 wk of achieving maximum dosage.
