In Short, The Case Is As Follows: On The Day

Question: In short, the case is as follows: On the day 18/05,I entered in a hospital
with diagnosis Infectious cellulitis in the right leg and for this reason
my leg was operated by cutting my calf (the gastrocnemius)on both sides to get rid the liquid that had been
accumulated in the same calf as a result of the infection.
Everything was going well with both the antibiotic treatment and the healing
of the surgical wounds until it was time to discharge from the
hospital on 06 / 06.Then the surgeon removed the surgical clips
with the thread that joined the two edges of the wounds and let me go home. After 4 days when arived the first consultation in the ambulatory
and the nurse removed the bandage it was clear that the skin on the inside of the
leg was very open. I have to make it very clear that ONLY the skin that covers
the muscle had shrunk leaving part of the tissue open, but the muscle itself
was perfectly healed. The surgical wound in the other part of the
calf was also very well healed. Several days later(14/06) they called me again and put
 a gadget: https://www.smith-nephew.com/latin-america/productos/care_avanced_heridas_a_z/pico/
(PEAK from XXXXXXX & Nephew) that, according to them, could contract the edges of the skin by closing it
thanks to the negative pressure produced by said device. The problem is that when I was
on the 21/06 to consultation after a week of use of this device it became clear that the
effect was contrary to what was expected, because the skin seemed even more open.
It worries me how the healing of this wound will end and how it can heal
such large space (1-1.5 inches width). The surgeon says that the area between two edges of the wound is
going to fill with skin, but I have doubts and I need the opinion of an expert in this matter. Greetings and thanks in advance!

Brief Answer:
Can heal conservatively or with surgical closure.

Detailed Answer:
Hello,
Thanks for posting your query.
I have seen the attached image. There is a large skin gap in the area of infection. This can heal conservatively after all the drainage from the wound dries up. But it will take a time of 6-8 weeks to heal properly on its own.
The other option is to go for surgical closure of wound with secondary suturing and skin grafting. This will help in healing in 7-10 days. You can discuss regarding these options with your doctor and choose the treatment best for you.
I hope this answers your query.

Thanks for the for the fast answer dr. Tayal! I would like to ask you one more thing. What do you think about wound closure systems like Dermo Clip, Close X and Zip Stitch?Do you think that the use of these devices could help the wound heal faster by closing the edges of the broken skin?I want to emphasize, that I do not have any pain in the leg or inflammation or any other discomfort.

Brief Answer:
It is better to avoid using these.

Detailed Answer:
Hello,
Thanks for writing again.
The closure of wound with the devices like Dermo clip, Close X or Zip stitch can cause an extra stress on the suture line and chances of infection are also present. It is better to avoid using any of these.
Hope my answer is helpful.
Regards.
