Question: I'm 31 weeks pregnant with an ALP of 324. I have Ankylosing Spondylitis and a history of cholestasis of pregnancy. Should I be worried?

Welcome to Health Care Magic.

Ankylosing spondylitis or spine inflammation you may not be able to receive epidural pain medications during delivery.

During pregnancy ALP can be normally increased by 2-3 fold. There is a small possibility that the increased ALP level is due to Cholestasis of pregnancy.

Ankylosing spondylitis does not affect your baby. It can affect the mode of delivery. If arthritis has involved your hip joints, then you need to be delivered by cesarean section. If there is fusion of spine or spine inflammation, you may not be able to receive epidural pain medications during delivery.

Cholestasis pregnancy may increase the chances of your baby being born early.
There may also be a slightly increased risk of fetal death in late pregnancy.
You need to repeat liver function weekly.

Your doctor will perform weekly ultrasound with biophysical profile and non stress tests from 34 weeks pregnancy to reassure you about baby's health.

Labour is induced once you complete 37 weeks of pregnancy to avoid the possibility of fetal death in late pregnancy.
