Is Actrapid The Right Medicine For High Blood Sugar Levels?

Question: My father XXXXXX suffered a brain stroke on 23rd July'14, he was immediately taken to a nearby hospital.
Concern doctor gave him following medicines
1. Vita comf plus, Mecobalamin niacinmide pyridoxine hydrochloride
2. IP Meropenem, Pantopazole injection
3. Neurotol, Mannitol &Glycerin
4. Phenytoin tablets IP 100mg Eptoin
5. Radipan-40, Pantoprazole injection
6. Cefpodoxime & Ofloxacin tablet, sefomax
7. Citicoline and piracetam tab, storax pr
8. Citicoline injection 250mg/ml, Storax
9. Olanzapine tab IP, Oleanz 2.5
10. Enteric coated Rabeprazole Sodium & Domperidone cap, Rabsig-DSR
11. Atorvastatin tab IP, Tonact 10
12. Liv-R specific
13. Magaldrate & simethicone oral suspension USP

Along with insulin dose. Later he was released with medication as mentioned above.
He also suffered paralytic attack disabling his right side arm, leg, waist.
After almost 2 1/2 months there is now little movement of arm, legs etc as he is been under physiotherapy. But sugar level is still high which may be hindering his recovery. Now 3time insulin Human Actrapid 40UI and sugar level is coming down slowly. Please suggest whether complete recovery is possible through other surgical or any other methods.
Presently doctor pescrived him following medicines after 1month recheck.
1. Livstapin
2. Filiz-S-10
3. Lonazep 0.5
4. 4. Phenytoin tablets IP 100mg Eptoin

I have noted your father's history, CT scan reports and current treatment.

He is on correct treatment.

Physiotherapy should be continued, however, it should be noted that recovery would be slow and may be partial.

There is not much role of other forms of treatment, such as surgery, etc.

My father is at home now taken care by my elder brother and mother, myself residing and working in XXXXXXX
At home they makes him sit on chair for an hour in morning and at evening, regular bathing/cleaning.


Some problem noticed at home

1. Food not able to swallow properly as compered to initial days of treatment.
Food used to get struck though food is meshed properly. Now a days he takes water for swallowing the food(even it gets
struck which make him makes him coughing like action). He lost lot of weight and takes less food, any medication to treat this problem and increase his hunger.
Kindly also suggest food chart likely to help him feel good and gives him strength.

2. Though he is on sleeping pills(night time) as per doctor prescription, he remains in unrest state for whole day(even when TV is on) and
not able to often recognize people, object, etc, we feel it is getting worse day by day. How to treat this problem.

3. He feels pain in right thigh, hip bones when physiotherapy is done, it pains him terribly at that time he
responds very spontaneously to the physiotherapist asking him for mercy and begging for giving rest.
What could be the reason and any possible treatment? And is it going improve?

4.Some other observations
a). His speech is also getting unclear, at some day he speaks clearly and some day very lightly.
b). He is not able to feel that he is urinating(hence he is on external chethetar) but for stool he is able to tell. How this can be improved, please suggest.

Doctor I would also like to ask how worse the stroke is seeing the reports, and can it repeat again in near future. Same time what short of diet he should be taking for health recovery.

My replies are below.

Thank you for getting back. I have noted additional problems and the replies are below:

1. If there is a swallowing problem, there are two methods of increasing food intake: One is by Ryle's tube, where food is inserted from nose to stomach, and the second is a procedure, PEG, where pipe is inserted directly into the stomach by a minor procedure. There is no medicine to improve swallowing.

2. For restlessness, qutiapine tablets can be used.

3. Pain is common during physiotherapy for stroke due to stiffness of muscles and nerve-pain. Medications such as baclofen and gabapentin would help. Also, volini or Moov gel can be applied on the affected areas.

4. Speech and urine control would improve slowly over time. There are no specific medications for them.

Stroke is of moderate severity. If his blood pressure and sugar are kept within control, then, new stroke would not occur.
