Question: Hello ,I have hyper pigmentation for yrs , extremely fatigued , tried all all kinds of skin treatments , products , Lazer , skin needling made it even worse , I am tired all the time , can you help with this skin discoloration, ? Will glutithiobe help ?I have been to dermatologist for yes , it lessens and comes back agan ,my hair is constantly thinning out , I have bad breadth , I do have a lot of stress

I have gone through your complaints and understand your concern.
Melasma is very resistant to treatment in some persons.
Gluthathione is not found to be very helpful in persons with melasma.
Though you can give it a try.
I usually recommend my patients to apply triple combination cream containing tretinoin, mometasone and hydroquinone on the pigmented patches which are tapered over a period of time along with tablet tranxaemic acid 250 mg twice daily.
These are prescription drugs and should be taken under the supervision of the doctor.
Besides this frequent application of a broad spectrum sunscreen every 3-4 hours during the day time is recommended.
For thinning of hair apply minoxidil 2 percent lotion twice daily.
This will take care of it.
For bad odour you need to consult a dentist.

Hello Dr ,I was hoping you to say it's my digestive issue , which is not getting nutrition to my hair, that is why I am so tired ,never hungry , I am always home , so 2 3 times sunscreen does not make sense to me , I do not go out during the day , my teeth are in very good shape I visit my dentist at least 2 times a yr , I have no gum or teeth problem, I practice very good dental hygiene, bad breath is from stomach issues I believe, from digestion problem . I have tried Minoxidil, my scalp breaks out and itches bad ,so I cannot do that , you mentioned the acid pills ,what is that for ,I am in the US , cannot get any medicine without prescription, unless it's over the counter , my Dr will not prescribe meds suggested by another Dr , are you in the US ? My mom ,my only sister does not have melesma , this all started from a product that made my skin itchy and I scratched my face during sleep , it was in one cheek , tried to get it out , .by microdermabrasion , did not work, peels Lazer ,my skin feels the pain or heat , it kept getting worse. I thought I would get a different outlook but everything you suggested has been done ,no results.

Sunscreen has to be applied even if you are indoors as even visible light has an effect on face.
For hair if you are not ok with minoxidil you can go for hair serums containing capixyl and other nutrients.
It will help in dealing with your hair problem.
Also take hair supplements containing biotin.
Tranexamic acid pills are not for acidity. They are the new treatment option for cases of resistant melasma.
You can discuss with your doctor regarding the possibility of putting you on tranxaemic acid.
