Detailed Answer:
It is not the purpose of this online forum to give exact prescription guidance.
But I can broadly guide you as to the best treatment approach.
When I see someone like you in my practice, I ask them to see a dietitian to learn about healthful eating and also increase physical activity. After 1 week trial with this, if glucose targets are not achieved then medication like metformin or glyburide or Insulin are necessary. The choice of treatment depends on many factors and one of them are the blood glucose levels at the time when decision has to be made.
So you will need to see an endocrinologist in-person for a comprehensive assessment. In general Insulin comb 25 are not ideal and cause fluctuations in blood glucose levels.
