Is HIV 1 And 2 Antibodies Test Conclusive At 91 Days Post Exposure?

Question: Hello doctor I had a unprotected oral sex on the night of 3 January 2015. It is possible that tip of penis might have been touched by hands with secretions.i tested for HIV 1&2 antibody p 24 combo test at Dr Lal path labs at 10,15,30,52,72 days and also I got tested on 4 April 2015 considering it as 91 day. Is my calculation correct for 91 day. My tests results were non reactive with last test having a value 0.07 non reactive i am infected with hiv. Do i need a retest at 6 months.

I have done a combo test at different time intervals that is after 10 day,15 day,30 day,52day,72day and at 91 day. Are they not conclusive. Combo test detects for p 24 as well in entire three months the p24 was not detectable. Is it not conclusive. Why most information says 90 days as conclusive.

Please find the clarifications below.

1. HIV virus is found in XXXXXXX in type 1 and 2. The HIV 1 and 2 antibodies test shows the presence of antibodies depending on the type of infection a person a patient has. The p24 test shows presence of HIV 1 virus protein.

2. The HIV antibodies can take 30 days to 6 months to form in most patients. In XXXXXXX 95 percent to 98 percent infection is HIV 1 and 2 to 5 percent is because of HIV 2.

3. There was no need to do the test so many times. It was enough if you did the test at 3 months and again at 6 months. Your test is 100 percent conclusive for no HIV 1. The last test at 6 months is only to confirm absent HIV 2 antibodies.

4. It is said that 3 months testing by HIV 1 and 2 and p24 combo is conclusive for HIV 1. However there is a small chance of HIV 2 infection which will not be conclusive on combo test done at 3 months. HIV 2 is always tested by antibodies.

5. Keeping in mind both HIV 1 and HIV 2 it is best to do the antibodies test one last time at 6 months to make it conclusive that you do not have any type of HIV infection.

6. In USA, there is almost no risk of HIV 2 therefore they say HIV antibodies and p24 combo test is 100 percent conclusive at 3 months.

I understand it now thank you. I would like to ask what are my chances if being infected with hiv 2. Will I survive. Am i free from hiv 1. Most people develop antibodies to hiv within three months but immune systems of people in XXXXXXX or us are Same if they develop antibodies within three months what will be the reasons for developing antibodies after three months for people living in XXXXXXX is it due to hiv 2. Further are there chances that I will infect my 7 year old son because he shares same bed with me or due to some accidental exposure. I have seperated my tooth brush razors etc.

1. There is about 1 in 50000 to a lakh chance of HIV 2 and this is 0.00001 percent. It means 99.9999 percent you do not have HIV 2.

2. You are free from HIV 1 and this is good news.

3. The last 0.00001 percent can be ruled out by doing 6 months test for antibodies.

4. Development of antibodies takes place in more than 95 percent people having infection within 3 months for HIV 1 and 2.

5. There is no such thing as HIV 1 antibodies will develop earlier or later. Anti body development depends on immune system of the person whether they are in XXXXXXX or USA without any difference.

6. There is no risk of infection to your 7 year old son. There is no chance of accidental exposure due to sleeping on same bed. Please know that chance of you having HIV is almost nil and the test is only to follow rules as recommended by health guidelines and get peace of mind.

Thanks doctor for your response and patience and bearing with me I would like to ask what if I am infected with HIV 2 . Will I survive. Do medications work for HIV. What will be average life time Further will I expose my son accidentally if he has a small cut/ injury on knee and it comes in contact with me or my nail scratch his skin while sleeping with me or catching him running. also pls advice why my doctor at Fortis said that no further testing is required. Even a doctor at health care magic told concluded the results after 15 days. Further will you conclude the result at 6 months or after 6 months. Also 95 percent of people will develop antibodies to hiv1 or 2 within three months are there some reasons for left 5 percent taking more than 3 months.

1. First of all you will never get HIV 2. Oral sex HIV risk is very less and HIV 2 is even less and therefore your thoughts about survival and treatment of HIV is beyond imagination. You will never get HIV from your oral sex incident.

HIV is a condition which has a lot of awareness these days. I have had patients who are HIV positive and they are normal just like other people. With medicines and precautions they can survive for more than 15 to 30 years and even more.

2. The 6 month rule is compulsory for those having unprotected vaginal sex. Since you have had unprotected oral sex therefore the risk is almost nil and not a concern.

3. I suggested 6 months to double confirm and clear all doubts. I am very strict in giving advise to my patients and tell them to follow international guidelines. The other doctor at Fortis and healthcaremagic have said it almost right but I am going one step further for your benefit and to give you 100 percent peace of mind.

4. Your son does not have any risk if he has small cut injury or scratch that comes in contact with your nail.

5. At 6 months I will conclude that you do not have HIV 1 or 2 infection. This is following international guidelines in HIV testing. The 5 percent who take more time to have antibodies to HIV 1 or 2 usually have a delayed immune response and therefore take more time. Some studies have also shown that 99 percent of people have antibodies to HIV after 3 months. Doing a 6 month test is 100 percent safe for people in XXXXXXX and XXXXXXX because p24 protein is not there in HIV 2.
