Brief Answer:
For 45 kg meftal will be OK

Detailed Answer:
Hello

Thanks for the query

I understand your concern. Crocin contains paracetamol and it is the safest antipyratic. It can be given at an interval of 6 hours.Tepid sponging also helps in bringing down the temperature.
If fever is not responding to these measures, you can give mefenamic acid(meftal). In Meftal P, 'p' stands for pediatric. It contains 100 mg of mefenamic acid while meftal contains 500 mg of mefenamic acid.
It is always better to give medicines according to body weight. For 45 kg meftal(500 mg of mefenamic acid) is OK.
You can give meftal at an interval of 8 hours.
In case of such high temperature I would suggest you to get him evaluated by pediatrician to look into the cause of fever and to treat the underlying cause.

Please let me know if you have any more doubts.

Brief Answer:
Please provide detail history and attach the reports

Detailed Answer:
Thanks for follow up

Most common cause of fever in children is some viral infection.Fever may be the only initial symptom in case of viral infection and other symptoms like cough/cold or loose stool may follow after 2-3 days.
In viral fever temperature usually starts declining after 3-4 days.I would like to know if the is fever declining or increasing in severity.
Other common causes are malaria, typhoid, dengue or some other bacterial infection.
As already fever is there for 3 days I would like to get CRP(c reactive protein)test done to see whether it is bacterial of viral if the fever shows the same trend in next 24 hours. The value of CRP get increased in bacterial infection and antibiotic treatment will be needed in that condition.

I won't get worried for borderline RBC count if other values are normal.
I request you to attach the reports so that I better able to help you out.
