It is not immediately clear to me how long this pigmentation has persisted.
You seem to suggest you have tried this epifager cream for 3 mths sans effect. If so, no harm in stopping this & shifting to the suggested melaglow cream.
The effectiveness or not would depend on a multitude of factors not least if this cream contains hydroquinone; are you allergic to hydroquinone & the reason for the pigmentation developing in the first place.
It is a nigh impossible task in pigmentary issues to predict outcome & if so the duration it would take to resolve but I would hazard a guess in that if you do not see improvement in 2-3 weeks, then please do take a second opinion. Each person has hi/her personal opinion but I am not sure how applying it for 1 hour specifically at night will help. I routinely ask my patients' to keep these depigmenting agents on for at least 3-4 hrs before sleeping for best contact & effect. Then again a chemical peel may suit you better again depending on the reason & duration & 'depth' of the pigmentation & your expectations therof.
Still, I am a firm believer that you should follow the advice of the doctor you have been to & don't second guess for now.

Again, from the ingredients you describe, it seems feasible to be used on a long term basis.
However, I have an old school mentality when it comes to looks & beauty: "if it ain't broke, don't fix it" ...in other words, if it has helped her & done the job, perhaps best not to overdo it.
But for best results again, let her take the medication & herself to a dermatologist nearest her. If in XXXXXXX region, she is most welcome too to come across to XXXXXXX Hospital, off XXXXXXX road.
