Question: Male 56. PSA 4.25 Free PSA 13 What would you suggest? Cancer?

Hi,

I understand your concern.

PSA is a protein produced by prostate cells. It can circulate in the bloodstream bound to other proteins and free (unbounded).

We do measure PSA normally which is bounded with other blood proteins. Your level is 4.25; it falls within normal range. In the new guidelines, when a male is having PSA level between 0 and 10, we do order free PSA in order to decide whether you need biopsy or not.

As your PSA is 13%, that is not suggesting cancer although there is a 28% of chance for cancer.

Before precipitating into conclusions, I would like to know if your doctor did a digital examination before the blood was drawn to run PSA test. This is important.

Next, abdominal ultrasound is necessary to run to determine whether your prostate is enlarged. Because enlarged prostate and inflammation can also cause such slight raise in both PSA results.

Once again, I do not think of cancer this time, but should look for enlargement? inflammation/infection of prostate? or any possible prostate surgery?
