Is Romilast Safe For Treatment Of Respiratory Tract Infection In A Child?

Question: My 1 year 9 months old daughter is having cold problem from april this year. I consulted a child specialist and he advised X-Ray of chest and Blood test. The X-Ray report was as sided patch+ and blood result PLT 2.9 Lakhs/cumm. Final diagonsis was Bronchopneumonia. The Dr. admitted my daughter in hospital on 22/04/2014 and starts treatment with Antibiotic Altacef , Antipyretic and Nebulised bronchodilators. She was dischaged on 26/04/2014 after the 14th does of antibiotic. After discharge Dr. given an antibiotic Clavet for 5 days (Daily 2 times), Ascoril for 5 days (3 times). After this does we consult the Dr. and he said she has some problems stiil. Dr. advised to take Romilast 4mg for 14 days. Is it advisable to give this medicine to my 1 and 9 months old daughter.Please advise.

Respiratory tract infection is quite common in children especially in summer season. On occasion it could lead to involvement of lower respiratory tract and leading on to Pneumonia. As this is single episode of Pneumonia , which is not related to airway Hyper reactivity like Asthma, it does not require any prophylactic drugs for prevention.
Romilast ( Generic name Montelukast) is Leukotriene receptor antagonist . It competitively inhibits the Bronchoconstriction , increased vascular permeability ( seen in cases of asthma). End result is Bronchodilatation resulting in opening up of blocked airway and suppression of Bronchial Hyper reactivity and airway inflammation. Romilast is mainly used prophylaxis of Asthma, as it prevents future attacks.
In my clinical practice I never use Romilast for patients with Pneumonia especially in less than 2 years of age. Every Physician have their own beliefs.
