Thanks for your query.
Allopurinol reduces Uric Acid levels in the blood and is used as a preventive treatment for
gout. It has NO role in the treatment of an acute attack of gout.
Acute attack of gout is treated with anti-inflammatory drugs and
Colchicine.
Since you are allergic to anti-inflammatory drugs, you may take Colchicine (.5mg) as follows :-
Initial dose = 1mg (that is, two tablets) and thereafter 0.5mg every 4-6 hours. Continue it till the pain and swelling is relieved.
However, the possibility of you being hypersensitive to Colchicine, cannot be predicted. So, to test for
hypersensitivity, you can take 1/4 tablet initially. If there are no indications of hypersensitivity, you can then take it in doses as prescribed above.
Allopurinol SHOULD BE STARTED (and continued long-term, with regular monitoring of Blood Uric Acid levels) only when the acute attack has been cured.
Injection Synacthen is a steroid.
Steroids have significant anti-inflammatory effects and also have a potent anti-allergic effect. It can therefore be taken safely along with Colchicine.
