Brief Answer:
Medical dissolution for stone. Need to increase fluid intake.

Detailed Answer:
Good day and thank you for being with healthcare magic!

A small kidney stone measuring 4mm is what we consider a silent stone. It is small enough when it falls down the ureter it would easily pass out and usually we treat that with medical dissolution like potassium citrate tablets 3x a day.

In terms of his other complaint where a hole is always produced where his penis touches his underwear could be because of a highly concentrated urine. This could also be the reason why he has a stone in his kidney.

I suggest he should drink at least 2-2.5 liters of water a day. This would help dilute his urine and prevent the stone from growing.

I hope I have answered your question satisfactorily and please consider a five star rating for my answer. Thank you very much.
