Is A TSH Reading Of 2.6 During 30 Weeks Pregnancy A Cause Of Worry?

Question: my wife is 30 weeks pregnant and she is suffering with hypothyroidisum and her current thyroid values are TSH 2.6, t3 222, t4 12.9, are these values are fine ???

Hello

Current TSH level is within normal limit.
Total T3 and T4 level are mildly increased.

Normal TSH level should be 0.5 - 4.70 µIU/mL and her report is 2.6,so it is normal.
Normal T3 level should be 80-180 ng/dl and her report is 222,so it is mildly increased.

Normal T4 level should be 4.6-12 ug/dl and her report is 12.9,so it is also mildly increased.

Free T4 and TSH level are most important factors during pregnancy.Since her TSH is within normal limit,mildly raised T3 and T4 are acceptable.

She should take thyroid tablets daily.

We haven't done any free t3 and free t4 test till now from the day she conceived to till now we are checking only tsh and we are closely monitoring every month and we are trying to keep this tsh under 2.5, so do we need to go for ft3 and ft4 test or is it ok to continue as it is. Before she pregnent she used to take 12.5 mg thyronorm but from the day she conceived to till now she is taking 25 mg thyronoram we havent changed the dosage yet..is it ok ??

Hello

TSH is most sensitive to detect hypothyroidism.
Her TSH level is fine and there is no need of further tests or dose alteration.
Free T4 is generally advised if TSH is raised.Free T4 is the form which is biologically available at cellular level.
She can continue 25 microgram daily dose of thyroid tablet.
