If you try to reduce the lump by pressing on it?

I do feel a slight increase in size while coughing. It doesn't seem to reduce at all when I apply pressure , and it is very firm as well. It is normally painless unless I am doing heavy lifting where I sometimes get a sharp, aching pain. I have had this for almost 2 years now.
