Is A Negative Test For Gonorrhea And Chlamydia Conclusive?
Question: I had un protected sex. 4.5 days later i had swab test for chlamdiya and gonerrha which were negative. 3 to 4 weeks after the exposure i started feeling mild burn in my penis, but no discharge. I got urinalsis which cam back with elveated pus cells. I was precribed antibiotics. Is the high pus cells indicating that my chlamdiya and gonerrha tests could be false negative?
Brief Answer:
Swab test for gonorrhea and Chlamydia are not so sensitive tests.
Detailed Answer:
Hi,
Welcome to HCM.
Swab test for Gonorrhea and Chlamydia are less sensitive. In a asymptomatic male it is around 50-75% only. No test is 100% sensitive. Culture and NAAT are reliable up to 95-96%. (http://www.bashh.org/documents/4490.pdf). Moreover the test is highly unreliable for Chlamydia by 4.5 days.
Presence of pus cells (above 5) in urine along with burning sensation during urination indicates that you are having urethritis. NAAT test will help to fix the etiology. Appropriate antibiotics will cure the problem. Your sex partner also has to be treated simultaneously.

If only i had anal sex my partner, my understading is that she will only get rectal infection. Is that correct, and if so, will the bacteria (chlamdiya or gonrreha) move from the rectum to the vagina?
So to make it simple, is her vagina going to be infectious and it going to influnc her reproductive system (since i did not had vaginal sex and only anal)?
Does rectal (chlamdiya and gonerrha) heal on their own? If yes, how long does that take?

Brief Answer:
Rectal gonorrhea and Chlamydial infections have to be treated.
Detailed Answer:
Hi,
Welcome back.
Yes, you are correct. If you had anal sex alone and if you have either Gonorrhea or Chlamydia your partner is likely to have rectal Gonorrhea or Chlamydial infection. Her vagina and reproductive tract will be spared from infection.
Due to faulty cleaning habits, one can herself can infect her reproductive tract unknowingly from the rectal infection. This possibility can not be ruled out.
Otherwise there is no possibility of a rectal infection moves to reproductive tract.
Spontaneous healing of rectal gonorrhea or Chlamydia is unlikely.

will 1 g azithromycin along side with 500 g Levofloxacin for 5 days cure rectal (gonerroha and chlamdiya)? My partner has a senstive stomach so ceftriaxone is not an option, and also it is very hard to provide injection treatment where I live, so what is the best course of antibiotics in this do you reccommend?
Brief Answer:
Consult your physician.Examination and investigations need before treatment
Detailed Answer:
Hi,
Welcome back.
1 gm Azithromycin and 500 mg Levofloxacin are enough to cure both Gonorrhea and Chlamydial infection. It is applicable for genital infection whereas rectal infection sometimes likely to relapse. So we have to follow up with tests for cure. If your partner is sensitive for Inj. Ceftriaxone, there is nothing wrong in taking Azithromycin and Levofloxacin. Anyhow everything should be taken under the supervision of a Physician after examination and investigations.
