It's definitely not hemorrhoid, if hemorrhoid burst /rruptures it leads to heavy bleeding. Presence of recurrent swelling which is pus filled and with blood suggest infective lesion. Possibility of Pilonidal Sinus needs to be considered. Exact diagnosis will depend on clinical examination.

1. Kindly let me know since how long has he been having these symptoms
2. First time when he had the blister , did it occur immediately after fall ?

Dear Ms
As the blister occurred 1 to 2 days after fall , it's unlikely to be due to trauma. Hematoma usually occurs soon after fall. Recurrent blister at same local suggest the following two possibility
1. Pilonidal Sinus
2. Fistula in ano
Exact diagnosis will depend on clinical examination based on exact location and appearance. I would suggest you to consult Surgeon for further clinical examination and assessment. These lesion are likely to recurr and require Surgical intervention in majority of cases.
