Question: I usually take 4U of insulin before meals...today my blood sugar is 251, I can not find my sliding scale chart, how much insulin should I take now before I eat?

Hello,

insulin dosage depends on the patient's insulin reserves and insulin resistance. Therefore the dosage is not the same for every patient, because different patients may handle more glucose with the same amount of insulin. Your doctor should have calculated the effect of insulin on your body and informed you accordingly (the scale proves that he did). Unfortunately nobody else has this information...

When doing fast-acting insulin before meals the doctor (and the patient) have to calculate the insulin dosage every time. Some insulin has to be given to correct a high level of glucose before the meal and some insulin would be used to manage the carbohydrates of the meal. The amount of insulin required for each of these tasks is different for every patient. If you'd like to go safe then you should try to contact your doctor. If you can't then don't add more than a couple of units more because it's better to have a high glucose level than hypoglycemia.
