Is Delayed Speech Normal In A One Year Old Child?

Question: Hi Doctor,

Our Boy, XXXXXXX in now 17 Months old. His weight and height are good and as per the standard growth chart. However, he is still not speaking. Ideally by this time babies would have 10-12 words in their XXXXXXX but our baby doesnt say anything except for blabbering and those words like 'TATA', 'ATTA'. He doesnt follow instructions often. He refuses to acknowledge when we call his name. But when he is playing with us, when you call you, he will immediately acknowledge and come to you. His hearing is all fine, we feel.

We do no know that every baby is different and they take their own time to achieve the mailstones. I wanted to check if we need to worry on this. What would be your recommendation?

Thanks for the response.

Below are the answers to your queries

1. What is the exact date of birth?
Its 26-May-2016

2. What was the birth weight?
Weight is 3.52 kgs at birth

3. What is the current weight and length of the kid?
Weight is 13.5 kgs and height is 82 cms

4. Did the baby cry immediately after birth?

Doctor said he cried immediately after birth but then when I got to see him after 15 minutes he didnt cry until for 5 hrs or so. His birth time is 3.32 in the evening. I saw him crying at 10 pm continuously

5. Was the baby breast fed in the first 24 hours of life?
Yes, the baby was breast fed within an hour of delivery

6. When did the baby attain social smile?
I think its in the 2nd month

7. When did the baby roll over (at what age)?
Towards the end of 4th month

8. At what age did the baby sit without support?
If I remember it correct, He sat at early 9th month without support

9. At what age did the baby stand without support?
He stood without support by 10th Month

10. When did the baby start saying mama - dada?
He doesnt say amma or nanna(telugu). He doesnt say anything except 'atta', 'tata'

11. Did the baby get seizures any time?
No Never

He started walking and then running in the 12th month.

Suggestions on speech delay

Hi...Greetings from XXXXXXX I am also Telugu. I am a consultant pediatrician at XXXXXXX Kamakoti CHILDS Trust Hospital, XXXXXXX

He is definitely having isolated language delay and otherwise he is normal in all other aspects.

We need to do -
1. BERA test for hearing assessment.
2. Check if he has tongue tie.
3. If there is a positive family history of speech delay - he will eventually start talking normally - nothing to worry.

Suggestions:
1. I suggest you are doing great by singing rhymes to him.
2. Do not use tablet or computer or TV or laptop for doing that.
3. Do not force him too much comparing him with others.
4. He might get benefited by speech therapy.

If he is seeing too much TV or tablet or laptop or cell phone - cut it to maximum 3 hours per week. Human interaction should be more and the main source of entertainment in this age.

Hi Doctor,

He Babbles a lot. My wife started speaking when she was just one year not sure of my case. As you have been answering all my queries right from his earlier months, would want you to suggest if we need to for speech therapy or wait a month or two.

Is there a way we can test from home if he has a tongue tie

He is now 17 months old.If you like us to wait a month or two, please suggest ways to make him utter words.

My wife frets if I say speech therapy. So wanted to try a few things at home first and then take it further. But all this would be based on your suggestion.

1. You can another 2 months.
2. At home you can try interacting more with him like, singing rhymes, slokams or even advertisement songs which he is interested in. But when he even shows interest initially in these or tries to utter any word or similar sound which you uttered, you can flatter him and praise him and give him a chocolate etc.
3. Do not force him too much comparing him with others.
4. Tongue tie can be diagnosed only by a doctor on direct examination please.
