Is Discontinuing Sotalol Safe While Experiencing Increasingly Frequent Episodes Of Mouth, Jaw Or Teeth Pain When Diagnosed With Prinzmetal Angina?
I have been taking Sotalol 80mg x 2 for around 12 years following Atrial Fibrillation which was cardioverted.
I have read that beta blockers are contraindicated in Prinzmetal angina and I am wondering if this has caused it.
Is it safe to stop the Sotalol after all this time?

Detailed Answer:
Hello Ma’am
After going through your medical details I understand your concern and I would like to tell you that some of beta blockers like Atenolol should not be used in prizmetal or variant angina as they can increase coronary artery spasm and will cause you more symptoms but beta blockers are used in combination to treat the symptoms of Prinzmetlal Angina.
Sotalol doesn’t have any contraindications in Prinzmetal angina and I will advise you , do not discontinue sotalol abruptly as it can cause acute exacerbation of coronary artery disease .
In my opinion you should get an Angiography to rule out if you have atherosclerosis present along with vasospasm, because in presence of atherosclerosis chances of cardiac arrest increases.
Treatment will vary for both the conditions. Prinzemetal angina can be treated only with medication but prinzemetal Angina with Atherosclerotic Coronary Disease will require more medication and procedures.
Hope to have been helpful, you can ask if you have more queries.

I understand that I need to be checked out Atherosclerosis and would need meds and procedures if found.
Hello Again
Detailed Answer:
Drugs for treatment of Prinzemetal Angina includes CCBs with nitrates and sometimes are given in combination with beta blockers.
Ankle swelling with CCBs varies person to person, you will have to check if you can tolerate calcium channel blockers and if you can than it will be very beneficial for you.
Yes, you should get an Angiography done to decide proper line of treatment, you can also talk about this to your treating cardiologist.
Hopefully this information will answer your query.
