Is Excess Saliva Due To Gerd? I Am Not Drooling

default
Posted on Sun, 24 May 2020
Question: Is excess saliva due to gerd? I am not drooling but I feel like my mouth is more watery than normal. I do have tightness in my throat for the last 4 months and feel it every time I eat or drink. Small bread crumbs or seeds stick in my throat and then I have to drink water to wash it down. I also have a weird pressure in my cheeks and around ears.

I also have pressure on my cheeks and top of my mouth and around my ears. I also feel my tongue is wet like constantly

I do have reflux. I have done an endoscopy, ph manometry and 24 hr impedence test last year and found little acid but nothing significant.

Brief Answer:
Anxiety or worm infestation.

Detailed Answer:
Hello and thanks for choosing "Ask aDoctor" service for your query,
Have seen your details and I appreciate your concerns.
I don't think that excessive salivation in your case is linked with GERD.
Two main reason can be-
1)Worm infestation- I would suggest you to take a course of Mebendazole 100 mg twice daily for 3 days.
2)Anxiety- Anxiety increases formation of saliva due to hyperactivity of nervous system.
please tell me about your anxiety levels.
Mark them on a scale from 0 to 10 where 0 is the least and 10 is very high.
Waiting.
Thanks!

Thank you for the reply, how about the other stuff, like the pressure and tightness in throat and the sensations in my throat?

Should I get a modifed barrium swallow done??

Does this sound like some neurological or autoimmune issue?

I am 29 year old male, dont smoke and no family history of any cancer or neurological issue.

Also, have this feeling of mucus in my throat which has stayed the same for the last 4 months. It differs in severity from day to day but have this every day. I did go and see an ent and he looked at my throat and saw mild inflammation of my lingual tonsils

Also, with excess saliva, I am not drooling or anything its just I feel I have excess saliva in my mouth and i feel its due to the pressure on my mouth and cheeks which is causing this sensation.

I have a fear that it could be due to some infection but then I might run a fever or have pain in my face right?

The other fear is like is it some neurological issue, like MS or other stuff.

Brief Answer:
Pressure tightness etc is due to acid peptic disorder.

Detailed Answer:
Hi again,
Pressure tightness etc is most probably due to acid peptic disorder. However if endoscopy has already been done then barium swallow have no role in your case.
I hope you are already taking medications for acid peptic disorder as prescribed by your gastroenterologist and therefore I have suggested you likely causes of excessive saliva formation.
What are your anxiety levels?
What medicines are you on?
Doses of medicines and duration?
Revert back.

I have not been diagnosed by acid peptic disorder

What are your anxiety levels? Very high

What medicines are you on? Pantaprazole(30mg) for the last 3 months

Doses of medicines and duration? 1 per day for the last 3 months

Do you think, this could be due to some autoimmune or neurological issue?

Hi dear,
Thanks for follow up.
As your Endoscopy and other tests are normal yet you feel reflux(You have penned down in your initial query) the other name to that reflux is acid peptic disorder. Its the acid and other contents of stomach which are refluxing back into esophagus. This reflux of esophagus causes contraction of esophagus(due to injury of cells) of esophagus. Cells of esophagus are not designed to sustain injury of acid.
It's not always possible to diagnose it. Even if endoscopy is normal and patient is having symptoms of acid peptic disorder/GERD patient is treated symptomatically.
You should start taking Pantopraxole 40 mg twice along with Domperidon 30 mg slow release to prevent excessive acid formation and it's reflux.

Anxiety itself is a cause of acid peptic disorder as well as formation of excessive saliva.Prescription medicine Clonazepam 0.5 mg should be taken once daily before going to bed.

Yes if anxiety levels are that high neurological component is definitely there.
Thank you.

I also have a constant tightness in my throat for the last 4 months and it spreads to my roof of the mouth and cheeks.

The stiffness makes my speech sometimes weird as it does not move as expected.
I am afraid if this is due to MS or some other neurological stuff.

When I am saying i have excess saliva, what i mean is that I am not drooling, its just a sensation of having excess saliva and I feel its due to the pressure/tightness in my throat and mouth.

Is this a sign of muscle weakness in my mouth and throat? I have not lost any weight, rather i am gaining weight.

What could be causing the tightness and pressure, could that also be due to the infection? Or some neurological stuff?

Detailed Answer:
Hello there,

Please try to understand when something is refluxed back from your stomach back into your esophagus or mouth it irritates the cells and tissues over there as they are not designed to withstand the acidity of your stomach...

When these tissues get irritated they get inflamed or swollen up this results in the feeling of tightness/burning sensation/Excess salivation ( to lubricate the damaged cells as a part of body mechanism).

Control your reflux properly by medication and you would feel much better. Pantoprazole 30mg is a baseline dose and secondly, it just prevents the excessive secretion of acid and has no role in preventing it refluxes back to the esophagus (that is done by Prokinetic drugs like Domperidone).

Any neurological deficit would have reflected back in endoscopy (in high definition camera your gastro could watch every moment of you upper G.I tract).

If you feel that it's infection go for a test called Complete blood count. They would draw 5 ml blood from your veins and if there is an infection anywhere Total leucocyte count would be increased(Fighter cells count).

As far as Multiple sclerosis is concerned it's an autoimmune condition affecting neurons of the brain and spinal cord. Clinically if you were a patient of M.S then why are the symptoms only limited to your upper G.I system i.e Stomach, esophagus, and mouth?

Usually, autoimmune diseases involve a number of organs at a time because almost every part of the body is supplied by nerves and M.S affect all of them at a time. So going by the principle common thing first kindly take proper treatment of GERD (acid control and acid reflux part both).
Get a stool test done to rule out worm infestation.

Finally, a complete course of antianxiety medication is advised as suggested earlier.

Are you following a bland less spicy low salt-based diet? Diet is the main factor when it comes to reflux. Unless you control your food things won't settle. The best and easiest way is to eat natural food cooked in less oil and use fewer spices...Avoid all processed and packed food.
No smoking/alcohol (if at all do).
Restrict tea and coffee.

Follow-ups are welcome,
Thanks

Without sounding like an idiot, could this be like a sign of some neurodegenrative disease like ALS/Parkinson or something even worse?

What are the signs of muscle weakness in throat and mouth?

Thank you answering my questions.

Hi there,
Resting tremors in hand, stiff muscles, difficulty standing, difficulty walking, difficulty with bodily movements, involuntary movements, muscle rigidity, problems with coordination, rhythmic muscle contractions, slow bodily movement, or slow shuffling gait etc are initial sign of most of the neurodegenerative disease and usually the progression is very fast unless medication is started.

Difficulty in chewing, loss of control of tongue,Drooling of saliva, weakness and numbness in muscle of face and mouth,Inability to eat solid food,Loss of deglutition reflex etc are sign of mouth and esophagus muscles.
Thank you!

Do you think what I am explaining is like weakness of the mouth muscles as I do have stiffness/pressure on my face/mouth?

I sometimes feel like the food stays on my throat and then I have to re-swallow them. Do you think it’s like a weakness of the deglutition reflex? And that’s why I have this weird stiffness/pressure on my mouth and throat?

Hi again,
My suggestion is that if you feel so then repeat endoscopy once again.
Via high definition camera motion of muscles from esophagus to duodenum could be seen hence any problem could be visualised.
Thanks!

Will a swallow study (MBS) rule out all weakness of the throat and the oro-pharyngeal phase and also rule out weakness in terms of swallowing?

I spoke to my ENT and he was like lets get this done and make sure that you dont have this issue and if its normal, then we can rule out all of my neuro-muscular/neurological fears.

Hi again,
Barium swallow studies were basically done to rule out any blockage in upper gastro intestinal tract.
Endoscopy is far superior to this test.
But if your ENT wants you can go for it.
