Is Hemoglobin Level Of 18.5 A Cause For Concern?

Question: Hi My Brother in law has a constant Haemoglobin 18.5, PCV 54 and high RBC Normal Platelets around 2L. Has no illness. Perfectly fit. Good Constant BP around 120-80. Used to smoke and drink a lot. Has stopped drinks and reduced smoking. Physically no problems. Gets good sound sleep also.

What could be the cause ?

He has polycythemia.

I have gone through your question. I can understand your concern.

He has high hemoglobin and PCV. It suggest polycythemia.

It can be primary polycythemia due to bone marrow disease with increase RBC production like polycythemia vera

Or it can be secondary polycythemia due to chronic hypoxia or eyrthropoitin secreting tumor.

He should avoid smoking, take plenty of fluid. Repeat her hemoglobin after 15 days.

If it remains persistently high then he should go for Erythropoitin level and JAK 2 mutation study. Then we plan accordingly.

Treatment depends on cause. Phlebotomy is very easy and economic treatment to control RBC count.

We are getting ready for Kidney Transplant. He is willing to donate one of his kidneys to his Siater. All other tests are fine. Will it be problem ? Or will he be able to donate kidney? Really worried now .

First of all we should search the cause of his persistent high hemoglobin then we should think about kidney donation. Because if he has some disease then will cause some problem later on.

Check function of both kidneys, erythropoitin level and jak 2mutation.

Because if he has polycythemia vera then reassasement is needed. It is not absolute contraindication but it need accement & risk benefit ratio should be taken into account.
