Question: want to ask a hematologist if an 18.8 WBC count a bad sign? Rheumatoid arthritis and Sarcoid (Remission for many years) are involved.

Normal white blood cell count generally ranges from 4 to 10 or 11. There are several different situations which might cause a chronic elevation of the white blood cell count. An inflammatory or auto immune disorder can sometimes contribute to elevated white blood cell count.

it is important that the complete blood count be assessed fully to see which type of white blood cell is elevated. There are several different types of white blood cells that are analyzed to be at the complete blood count.

another important factor is how long the white blood cell count has been elevated. If the white blood cell count has been mildly elevated for many months or years then this May be related to the underlying autoimmune disorder.

I think it would be very reasonable for the patient to have a formal consultation with a local hematologist who can evaluate the laboratory results and look at the blood cells under a microscope to see if there might be some problem with the bone marrow which might explain the elevation of the white blood cell count.
