Is Increasing Dosage Of Klonopin Advisable For An Intermediate Metabolizer For CYP2C19?

Posted on Wed, 19 Aug 2015
Question: Hi,
I am on Lexapro 5 mg, Klonopin 3 mg, Keppra 1000mg and Solian (amisulpride) 100 mg. My psychiatrist decided to give me a higher dosage of Klonopin, from 2 mg to 3 mg. I am an intermediate metabolizer for cyp2c19, so i just would like to know if this factor can delay a little bit the full effect of Klonopin: as far as i understand, since i am an intermediate metabolizer side effects are a little bit stronger (i guess from the beginning)and therapeutic effects come more slowly, since the med has to find his therapeutic window and it's more difficul than with a normal metabolizer. Thank you for all information i could have about this subject. XXXXXXX

Brief Answer:
The factor wouldn't affect the actions of the drug.

Detailed Answer:
Hello, Welcome to Health Care Magic

I can understand your concern. I hope you are aware of Cyp4502c6 enzyme system. It's the altered Cyp4502c6 activity that maters and not the metaboliser status. You are an intermediate metaboliser and this status wouldn't affect the metabolism of Clonazepam much. Metabolism in poor, rapid or ultra rapid metabolisers is effected more as compared to intermediate metabolisers. So no need to worry even if you are intermediate metaboliser that wouldn't effect the onset of action as well as side effects of the drug much. Slight dose adjustments as they have increased the dose would do the work.

So remain relaxed, the factor wouldn't delay the full effects of the drug. No need to be worried.

Thanks, Hope this helps you, if you have more doubts you can ask again.

Take care

Brief Answer:
No effects on onset and full effects of drug

Detailed Answer:
Hello again XXXXXXX

Yes, I know you are intermediate metaboliser for cyp2c19. This status wouldn't affect the action of Klonopin. No significant effects on its onset of action as well as on its full effects due to this status.

Thanks, still not clear please ask again.
