I am taking stool softeners I have one movement per day which is easily passed. My question is if the aching sensation is related to the fissure?
Brief Answer:
Answer given below
Detailed Answer:
Hi,
Yes, the dull ache and the intense sense of pressure are due to anal sphincter spasm which is a feature of anal fissure.
Sphincter spasm can be seen if digital rectal examination is attempted and it would be painful. Hence digital rectal examination is usually not attempted in anal fissure when the lesion is active.
Relief from this spasm is achieved with sitz bath and application of topical diltiazem at the anal verge.

Thanks for the follow up one last question I’m scheduled for a colonoscopy in a few weeks will this worsen fissure?
Brief Answer:
No
Detailed Answer:
Hi,
No, colonoscopy will not worsen anal fissure.
But if there is active spasm of the anal sphincter, the doctor may not proceed with the procedure as it would be difficult to introduce the scope inside and would be painful.

Will being sedated for the procedure make a difference?
Brief Answer:
Answer given below
Detailed Answer:
Hi,
Certain people feel discomfort during colonoscopy. This discomfort will not be there if you are sedated.
But sedation does not relieve the anal sphincter spasm. So even if you are sedated, the doctor will preform the procedure only if there is no anal sphincter spasm.

Also as I up fiber I seem to be getting very bloated and have lots of gas how can I aid bowel movements without all of the extra gas
Brief Answer:
Answer given below
Detailed Answer:
Hi,
Abdominal bloating is a common side effect of fiber.
So you have to adjust the dose appropriately to get good bowel movements and at the same time not get abdominal bloating.
Alternatively you can take a combination of laxatives ( fiber + prucalopride) and stool softener like Colace.
