The spotting per vaginum , post mirena insertion is seen usually for 2-3 months till the body adjusts to the regulated control release of progesterones from the mirena. If its been few years then occasional spotting is often due to stress induced hormonal imbalance which can cause irregular bleeding per vaginum. It usually subsides by increasing the water intake per day to average 3-4 litres. Water counters the progesterone withdrawal and is a natural de-stressor.
Also the fact that you are on anti-psychotics , the efficacy of mirena is decreased and can cause irregular shedding during the cycle.

Mirena is a intrauterine device and works as a mechanical contraception just like a copper IUCD where it prevents implantation of embryo inside uterus thereby preventing pregnancy. So contraceptive effect would remain as long as it is intrauterine.
