Escitalopram is an antidepressant. It is not wise to take them without a proper medical prescription. I strongly advice you against using it without consulting your doctor.

It's actually for my brother who lives in a place where it is not easy to get a clinical psycologist. He was prescribed the medication one year back by a doctor from Nimhans. He is facing the same kind of condition. Should we bring him again and treat him or continue the medicine prescribed earlier?

Antidepressants doses need to be adjusted to suit needs of patients at the same time make sure there are no adverse events. Further his current symptoms should be evaluated before starting trearment. This will allow treating to monitor his treatment response. So visiting a doctor before starting drug regimen is always advisable.
