Pregnancy appears to be highly unlikely if she was taking combined oral contraceptive pills containing estrogen>0.02mg. These pills are highly effective and provide >95% protection from pregnancy. Though she had forgotten one pill, she had followed proper procedure to compensate for that (taking pill next morning), so pregnancy is highly unlikely. The bleeding could be scanty menses/ withdrawal bleed.
Chance of pregnancy though <1%, can be easily ruled out by doing one home pregnancy test.
I can understand the stress through which you are going, but its better to kill the stress for once & all by doing one pregnancy test rather than living with stress. So please do one home pregnancy test if the event of missing pill had happened >3 weeks ago. If it happened <3 weeks ago, pregnancy test may not give true result & you will need to wait for few more days.
